Primary Care Network Support Service

We work in partnership with the Primary Care Networks in North Shields and North West North Tyneside to offer personalised support to patients who have a learning disability.

Our team will work with GP surgeries to identify patients who would benefit from some additional support to manage their health and wellbeing, particularly people who are anxious or have concerns about attending health related appointments.

Our Care Co-ordinators Lesley, Mandy and Steven support patients to attend annual health checks, make sure everyone is provided with accessible information and assist patients who may need to link in with additional health and care services.

Our Health & Wellbeing Coach Michelle supports and motivates patients to make healthy lifestyle choices, maintain long term behaviour changes and start to look after their own health and wellbeing.

Our Aims

  • To encourage and support more patients to attend annual health checks, medication reviews and screening appointments.

  • To identify patients who would benefit from health coaching and work with them to develop a personalised wellbeing plan.

  • To empower patients to look after their own health and wellbeing and assist them to access additional health and care services when required.

  • To ensure patients are provided with accessible information and and additional support when attending health appointments.
